In the hospital lobby the director arrives to confront Ruby after reports of an overdue medical bill. Bella and the director verbally attack Ruby — accusing her of sleeping around, getting pregnant, and even blaming her for Bella's brother's death — while the director insists he already saved your daughter and refuses further help. Insults escalate and the director orders Ruby to leave, threatening to throw her out. The episode ends with Ruby forcibly blocked: the director declares he won't let her meet him, leaving Ruby barred from the person she came for.
