Bree is rushed into the ER in cardiac arrest; doctors insist there’s still time for surgery. As staff prepare, a masked assailant bursts in demanding cash and threatening death, ordering Dr. Hart not to operate on "this bastard child." Dr. Hart refuses; the assailant breaks his hand, eliminating the only surgeon capable of the procedure. Bree’s mother pleads while a child's soft words echo. With minutes slipping away and the surgeon incapacitated, someone shouts "Kayla!" as Bree’s survival hangs unresolved.
